Salary Range $79,600 - $99,500 - $119,400
Coordinates care delivery to improve care quality, utilization of resources and reimbursement on behalf of designated patients according to practice models. Impacts patient care, satisfaction and institutional costs.
Essential Job Functions
Assesses individual patient needs based on practice models for high risk or complex care issues. Coordinates, facilitates and monitors care for patients throughout the continuum to ensure optimal clinical, functional, social, cognitive, satisfaction and financial outcomes. Oversees discharge planning and facilitates transitions to community settings. Intervenes, advocates and facilitates care changes on the patient's behalf. Acts as a liaison for multidisciplinary teams, external physicians, payors and community agencies. Identifies and implements strategies to improve processes, reduce costs and enhance patient care and business outcomes.
Required: Graduation from an accredited school of professional nursing.
Required: Three years of clinical nursing experience to include experience in coordinating care for high risk and complex patients.
Preferred: Two years hospital based Case Management experience.
Required: Current State of Texas Professional Nursing License (RN).
Preferred: Certification as a Case Manager.
